How It Works
Chronic Care Management
CCM is a Medicare program for patients with two or more chronic conditions. Practices can earn reimbursements by providing preventative care to patients each month, with ChartSpan’s help.
Patients will receive 24/7 access to a care line, help with refills and appointment scheduling, a comprehensive care plan, a care coordinator, and directions to community resources.
Quality Improvement Services
ChartSpan offers complimentary Quality Improvement Services to CCM clients. These services can help with MIPS scores, STAR ratings, or evaluations from ACOs.
ChartSpan’s Quality team consolidates quality measures into a streamlined program, identifies patient gaps in care, and addresses care gaps around vaccinations, screenings, and exams.
Annual Wellness Visits
RapidAWV™, ChartSpan’s Annual Wellness Visit (AWV) Software, makes it easier for practices to incorporate AWVs into already-scheduled appointments. RapidAWV™ can check patients’ eligibility and help them fill out their Health Risk Assessment electronically.
