Welcome to Continuum Care Hospice. Hospice is not a place. It is there when you need it, where you need it- whether it is a private residence, nursing facility or assisted living residence. The goal of the hospice is to provide palliative care that addresses the physical, psychosocial, emotional, and spiritual dimensions of service for the terminally ill as an integrated whole. Continuum Care Hospice is guided by the principles of responsive care, teamwork, leadership, and stewardship. Care to the patient and caregivers will enable those served to experience death with grace and dignity.
