MyPHA—short for My Personal Health Assistant—is a personalized advocacy and navigation program that guides individuals through the complexities of healthcare. Each participant is supported by a Personal Health Assistant (PHA), a concierge-like advocate and dedicated health coach, helping them understand their options and access the care they need, when they need it.
The primary goal of MyPHA is to proactively engage high-risk individuals—identified via Vision Hawk™ and the Whole Person Risk Score—and guide them to the most appropriate care path. This might include Advanced Primary Care (APC), a specialist referral, or a relevant point solution. Through high-touch support, MyPHA helps people take charge of their health, close care gaps, and achieve better outcomes while reducing costs.
