Omada for Hypertension (https://vimeo.com/668278680) focuses on improving members’ blood pressure control to help reduce cardiovascular risks. Applying clinical protocols recommended by the American Medical Association, the American College of Cardiology, and the American Heart Association, our dedicated health coaches provide 1:1 human support and resources for members with hypertension through connected blood pressure monitors, real-time secure messaging, group board discussions, as well as frequent and proactive check-ins, and who stay with members throughout their care journey. The coach may also consult with a hypertension specialist if needed. Additionally, members find real-time encouragement, empathy, and accountability in discussions with other people managing the same chronic condition in spaces and discussions led by health coaches. These groups are topic-specific and condition-specific, and provide inspiration and support to help members follow through on their care plan. Members stick with the program as they learn small, achievable decisions and shift their mindset as they realize behavior change and outcomes are possible.
