Rejuvenate supports patients through the transplant evaluation process by helping them understand requirements, optimize their health, and connect with transplant centers that may offer shorter wait times or better living donor strategies. We collaborate with healthcare providers to ensure patients are well-prepared for evaluation, addressing disease management, lifestyle changes, and social support needs to improve their chances of being waitlisted. We also assess social determinants of health (SDOH), including caregiver support, transportation, financial stability, and lifestyle factors, connecting patients with resources to improve their waitlisting opportunities and post-transplant success. Rejuvenate bridges gaps in care by working with the patient’s CKD management team, ensuring smooth transitions across pre-, transplant, and post-transplant phases. Through case reviews, shared electronic health records (EHR) when available, and multidisciplinary meetings, we optimize patient outcomes for the first six weeks. If elected, our Health2Hope team provides ongoing education and support to patients and providers, aligning care plans and addressing barriers to adherence. Patients contact Rejuvenate upon receiving a deceased donor kidney offer, allowing us to consult with them and their transplant team on suitability. For living donor kidney offers, we track transplant program acceptance practices and identify opportunities to optimize matches between patients, donors, and programs.
