Verily Group offers Lightpath, its advanced care management program. This program will pair a specialty clinical care team of licensed providers with the digital tools of apps, devices and artificial intelligence (AI) to offer a highly personalized and dynamic approach to care management. This approach informs member decision-making and drives improved anticipated health outcomes at scale. Verily Group believes in this technology-enabled methodology and is proposing a risk-sharing partnership model measured on delivering meaningful health outcomes for members.

Members enroll in Lightpath and have a personalized journey depending on their conditions, clinical needs, and progress towards their goals. Lightpath offers three program tiers for metabolic health (including diabetes) and weight loss management: Intensive, Improvement and Achievement.

All three tiers will focus on supporting long-term behavioral changes, including tailored nutrition, movement, medication adherence and mental and behavioral health strategies supported by our AI Companion. Clinical care team visits are included in our program pricing and do not incur additional charges. Depending on the tier and member need, pharmacists are available to help assess barriers to medication adherence.
