We engage these individuals with a whole person program (condition inclusive), blending lifestyle management and medication optimization personalized to each individuals needs and goals (many polychronic) – with unlimited 1-to-1 engagement with our MOBE (health) Guides and (clinical) Pharmacists. MOBE's program provides results across many chronic diseases including but not limited to: MSK, diabetes, heart health, pre-diabetes, weight management, tobacco cessation, digestive health, and stress management, with focus on health improvements in physical activity, nutrition, emotional health, motivation, sleep and mindset. By achieving health outcomes MOBE reduces expected healthcare costs (IP/OP/PROF/Rx) based on actual claims data and shares in the savings. Average 7.5% reduction in healthcare claims in the first year with 100% fees-at-risk.
