Proactive MD

Proactive MD provides a total healthcare solution for employers by partnering with them to offer onsite Health & Wellness Centers for employees, which are designed to elevate the standard of primary care, going above and beyond the typical boundaries of healthcare to meet each employee’s unique mental, emotional, social, and physical needs. Proactive MD’s care model is built on the foundation of strong physician-patient relationships and onsite Patient Advocates who personally guide and advise employees through the full healthcare system. Coupled with reduced-rate access to the nation’s leading specialists for complex and catastrophic diagnoses, the Proactive MD model dramatically improves the quality of care for employees while simultaneously reducing healthcare costs for employers. Proactive MD has also developed a first-of-its-kind “near site” facility, which allows medium-sized employers to provide their employees with all of the benefits of a Proactive MD Health & Wellness Center in a shared-employer setting.

Value Proposition

Proactive MD offers a fully integrated primary care medical home with a care team approach. Our care team includes a Patient Advocate whose unique role is to go beyond the traditional onsite model and truly integrate Care Beyond the Walls™ of the Health & Wellness Center through healthcare system navigation and education, dependent care resource education and navigation, medical literacy, community resource identification and acquisition, concierge appointment scheduling, risk-stratified engagement, and more. We provide patients with complex and catastrophic diagnoses access to the nation’s leading specialists at no cost to the patient and reduced case rates for employers through our exclusive Smart Care medical network. We set aggressive performance guarantees (20% of the total cost of the program at risk) based on outcomes including 3-year financial ROI, risk-stratified patient engagement levels, chronic condition management results, and overall service levels. Our physicians’ incentive pay is linked to the clinical outcomes of the members as well. By providing broad scope, comprehensive primary care based on a strong physician-patient relationship, Proactive MD becomes a true medical home and the gatekeeper for all employees’ healthcare needs, driving more appropriate utilization of the healthcare system, better health outcomes, and dramatic cost savings.
