We work with employers to offer Lyn to employees living with multiple chronic conditions. Once enrolled with Lyn, we provide:
virtual primary care and urgent care
care coordination, navigation and advocacy
medication management
integrated behavioral health care
social and community support

Lyn relieves/eliminates current pain points of managing health care to improve member experience and reduce total cost of care. This is supported by our ability to use data science that drives referrals to high quality and low cost providers. Further, we leverage predictive modeling to proactively manage member risk and improve outcomes, and we communicate with members using their channel of choice - whether it be phone, text message, email, video, website, and later in 2022, a mobile app.

Lyn utilizes a multitude of marketing techniques and partner with our customers to acquire members. Additionally, we leverage data science, behavioral science and design thinking to keep members engaged.
