As a care delivery organization, we understand that early signals are key to identifying clinical risks before they become costly conditions. We use data, including verification of benefits (VOB) and verification of eligibility (VOE) and claims to identify members on the path to becoming high cost and outreach to engage and offer support. We also have access to Admits, Discharge, and Transfer data, available to only care delivery organizations, to identify and engage with members at these key moments.
